Anyone can make a referral to Patrick Care. This includes support coordinators, LACs, healthcare professionals, family members, carers, or even the participant themselves. If you know someone who could benefit from our services, we welcome your referral.
If you are unsure whether you can make a referral, feel free to contact us, and we will provide the necessary guidance and support to help you through the process.
To process a referral, we’ll need some basic details about the participant, including:
- Full name and contact information
- NDIS number and plan details (if available)
- Primary support needs and goals
- Preferred services
- Relevant medical or behavioural information
- Referrer’s name and contact details
This helps us ensure we match the participant with the most suitable supports from the beginning.
Once we receive your referral, a member of our team will get in touch within 1–2 business days. We’ll discuss the participant’s needs, answer any questions, and begin the intake and onboarding process. If needed, we can also organise a face-to-face meeting to ensure a smooth and personalised experience.

We are best suited to individuals living in the Far South Coast or Snowy Monaro region (from Eden to Cooma) who are seeking services such as community access, supported independent living, respite, clinical support, or mental health support. We aim to commence services as quickly as possible, depending on availability and individual needs. In many cases, supports can begin within a few days to a couple of weeks after intake. Our team will keep you informed every step of the way to ensure a seamless and timely start.
